You understand that Nutrition Response Testing® is a safe, non-invasive, natural method of analyzing the body’s physical and nutritional status, and that deficiency, stressors, or imbalances can cause or contribute to various health problems. Nutrition Response Testing. uses the body’s natural reflexes as an aid to determine possible nutritional imbalances and develop safe natural programs and recommendations aimed at bringing about a more optimum state of health.
Nutrition Response Testing is a Trademark and Service Mark owned by Ulan Nutritional Systems and is used with permission. You understand and acknowledge that a Nutrition Response Testing exam can only be done in person by a properly trained professional. Any mention of Nutrition Response Testing on the Website, Programs, Products and Services, including testimonials, does not imply a similar situation or guarantee results in your case.
You understand that Rejuveo Cleanse™, Eating to Restore Balance™, and other Programs, Products, Handouts as may be released do not work well unless implemented as directed. Although designed to be easily understood and replicated, should you choose to do any program on your own and without professional guidance then you are less likely to experience positive results including but not limited to weight or fat loss, muscle gain, abdominal definition, more energy, less bowel irritation, or anything else.
There may be risks associated with participating in the Rejuveo Cleanse, Eating to Restore Balance or other programs for people in poor health or with pre-existing physical or mental health conditions. Because these risks exist, if you choose to participate in these risks, you do so of your own free will and accord, knowingly and voluntarily assuming all risks associated with such dietary activities. These risks may also exist for those who are currently in good health right now.

Payment, Cancellations, & Late Fees
As a convenience to you, Synergy Health & Nutrition, LLC currently has an online appointment scheduling system and can also receive online payments via Cash, Personal Check, PayPal and all major Credit/Debit Cards via the PayPal payments systems. Synergy Health & Nutrition, LLC websites do not store bank or credit card information.
Fees are payable in advance of each appointment or program start date unless otherwise agreed (Synergy Health & Nutrition, LLC may allow Corporate clients to send payment on receipt of an invoice). Where payment has not been received in advance of a session or program, Synergy Health & Nutrition, LLC is not obliged to provide the service or product.
Where receipts are requested by the client, they will be sent by e-mail unless otherwise requested.
Synergy Health & Nutrition, LLC reserves the right to terminate the consulting arrangement or insist on future cash transactions only.
Cancellation & Other Fees. I understand that my clients have busy schedules and I take pride in not keeping them waiting or keeping them longer than planned. Each session will start as scheduled and end on time. Please be on time. If you need to cancel or reschedule the appointment, you must do so 24 hours in advance; otherwise, that appointment is forfeit and will not have an opportunity to reschedule it.
Returned checks will incur a US$25 charge to cover bank fees and administrative costs. Appointments cancelled within 24 hours will be billed at the regular appointment rate. Unpaid charges plus any applicable fees are due on receipt of an invoice; payment not received within 30 days will incur a 2% per month late fee.
Termination of Services and Programs. You and/or Synergy Health & Nutrition, LLC have the right to terminate subscriber, member, and/or client relationships for any reason, including ending services that are already underway. Website and/or email subscription changes are available on your login page.
Returns and Refunds. There is no refund for Programs, Products, or Services that have begun and are underway, or for opened, non-resalable Products. Monies paid for future, unused Services may be refunded less any applicable cancellation fees or materials costs. If future services were purchased as a package discount, any refund will be calculated after subtracting the full, non-discounted rate.

Confidentiality. All Client information is kept private and not shared with third parties without your permission. Regardless of program type, Synergy Health & Nutrition, LLC will not share any Client information to any third party unless compelled to by law.
Arbitration, Choice of Law, and Limited Remedies. In the event that there ever arises a dispute between you and Synergy Health & Nutrition, LLC with respect to the services provided pursuant to this agreement or otherwise pertaining to the relationship between the parties, the parties agree to submit to binding arbitration before the American Arbitration Association (Commercial Arbitration and Mediation Center for the Americas Mediation and Arbitration Rules). Any judgment on the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof. Such arbitration shall be conducted by a single arbitrator. The sole remedy that can be awarded to you should arbitration result in such an award is refund of your Program or Service Fee. Without limiting the generality of the foregoing, no award of consequential or other damages, unless specifically set forth herein, will be granted.
This agreement shall be construed according to the laws of the State of Alaska. In the event that any provision of this Agreement is deemed unenforceable, the remaining portions of the Agreement shall remain in full force.
